Renal diseases of uncertain origin

Balkan nephropathy, otherwise known as Danubian endemic familial nephropathy is a form of chronic interstitial nephritis which seems to occur only in adult inhabitants of villages near the town of Vratsa, Bulgaria. The blood pressure is normal, there is *xanthochromia of the palms and soles, hypochromic anaemia and slowly progressive kidney failure, but no albuminuria. It was first described by a Bulgarian nephrologist, Dr. Yoyo Tanchev, and its aetiology remains uncertain, but the current speculation is that it is due to the consumption of aristolochic acid (8-methoxy-6-nitrophenanthro[3,4-d][1,3]dioxole-5-carboxylic acid). This substance is derived from various plants of the Aristolochiaeceae family, known to be carcinogenic to the epithelium of the urinary tract. Aristolochic acid may be derived from these plants, which common in the Vratsa region, and it is suggested that its seeds may contaminate the local wheat used for bread making (Grollman et al. 2007).
